NOW is the time for 6th grade parents to bring an updated copy of your student’s immunization record to the SCHOOL NURSE---IF YOU HAVE NOT DONE THIS YET.
___Tdap (tetanus-containing vaccine) - within last 5 years
___Varicella #2-Exception: if student had previous illness- must provide written statement from physician or parent
Please provide documentation to the nurse for the shots listed above. Letters were mailed home to students in 6th grade who need shots to start school next year. If you have questions please call Anna Hudnall RN 903-262-1508. Remember students that do not have their immunizations up to date will not be able to start school and will not have a schedule at the Back to School Bash in August. Thank you.
The Northeast Texas Public Health District will be administering back to school immunizations at the middle schools shown below. The month of August is always a very busy month for getting kids updated on their required school immunizations. By taking advantage of these clinics, you will not have to be the ones to sit for extended periods of time, waiting for a vaccination that you could have already obtained. “DON’T WAIT, VACCINATE"
04/29/2014 Boulter Middle School 4:00pm-6:00pm
05/06/2014 Stewart Middle School 4:00pm-6:00pm

Those kids who are on Medicaid and CHIP will need to present their cards at the time of service and there is no charge for the vaccines. Those who do not have any form of insurance will be assessed a $10.00 administration fee for each vaccine at the time of service. This payment can be in the form of a check or cash. For more information, please contact the NET Health Immunization Department at 903-510-5604. Sylvia Warren, RN NET Health Immunization/TB Director
